Tommy John Surgery/Ulnar Collateral Ligament (UCL) Reconstruction: Episode 131

Tommy John Surgery, an Ulnar Collateral ligament reconstruction of the elbow, is named for Tommy John. A man who pitched 26 Major League Baseball seasons from 1963-1989. He was the first pitcher to undergo the procedure in 1974 and was able to pitch another decade and a half.

Tibial tubercle osteotomy (TTO) is a surgical procedure that is often done to help individuals who are have patella instability or are dealing with osteoarthritis. This procedure moves the attachment point of the patella tendon.

Patella Dislocations make up between 2-3% of all knee injuries. They are most common in young female athletes and are usually due to a number of factors: genetics, mechanics, activity and trauma.
Piriformis syndrome is a common diagnosis for pain in the buttock. It is often attributed to issues related to the sciatic nerve but that’s not really the cause. In more than 80% of cases it has to do with some kind of trauma or repetitive motion not an anatomical difference of the piriformis muscle and sciatic nerve.

A capsular corner injury of the knee is not often talked about but is often present for individuals who are dealing with instability of the knee and are having a more difficult time returning to activity. We’re talking primarily about posterolateral and a little bit about posteromedial corner injuries of the knee.
The Partial Articular Supraspinatus Tendon Avulsion better known as a PASTA is the most common tear of the rotator cuff. It is present in up to 37% of the population and accounts for 90% of all partial thickness rotator cuff tears.
In this episode: Bunion is one of the more apparent foot changes that can occur. This is usually due to shoe wear and is more common with certain activities. Good news lots of it can be modified through an exercise program that focuses on your feet.
Lumbar strain also known as weight lifters back, low back pain, pulled back, my back went out, strained back and lumbago is a really common issues. It is likely that everyone will deal with this at some point in their lives.
Corticosteroids are used to treat a range of issues. From asthma, lupus and organ rejections too rashes and inflammation. They have been hailed as a wonder drug and for some people they are but there are some side effects to we aware of.
Groin injury is an issue that most commonly affects athletes that participate in kicking, skating and change of direction sports. Most groin injuries are mild and result in a short period of discomfort and inability to participate.
Paget’s Disease causes bones too soften due to over activation of osteoclasts. Osteoclasts are responsible for breaking down bone. In response to this the body attempts to quickly lay down new bone. Which causes enlargement of the bone, typically the spine, skull and long bones are most affected.

Fibromyalgia (FM) is a complex condition that can limit all facets of life for a person with it. The exact cause of FM is not fully understood and therefore medical treatments have been slow to develop and those available have varying levels of success.30 Pharmacological treatment is common for FM, however literature is beginning to surface that suggests the effects of physical therapy (PT) management may be as effective or perhaps more effective at reducing the symptoms.31 The effectiveness of PT management is based in large part upon the person with FM buying into the plan of care that unfortunately at times may make symptoms worse.

In this episode: Posterior cruciate ligament (PCL) is injured though either a sharp posterior translation of the tibia or a hyperextension of the knee. It is rarely injured in isolation. Upwards of 95% of PCL injuries occur with another injury.
In this episode: Lateral collateral ligament (LCL) injuries are rare. It is a round thick ligament on the outside of the knee. There is almost no data surrounding injuries to it in isolation. Even when a horrific injury like a knee dislocation occurs the LCL is often not completely ruptured.
In this episode: The medial collateral ligament (MCL) is a flat ligament on the inside of the knee. It is the most commonly injured ligament with some studies finding that it accounts for up to 10% of all injuries to the knee. Recovery time varies by grade, ranging from 10-60 days on average.
